Bone-on-Bone Knee Pain (Advanced Osteoarthritis)

“Bone on bone” describes advanced knee osteoarthritis where joint cartilage has significantly thinned, often visible on X-ray as reduced joint space and bone spurs. Common symptoms include deep joint pain, stiffness after rest, swelling with activity, and discomfort during walking or stair use.

Pain severity does not always match imaging findings. Many people with advanced osteoarthritis, aka bone on bone, experience fluctuating symptoms influenced by joint compression, dehydration, inflammation, nerve sensitivity, and limited movement.

In regions with hills and uneven terrain, such as Asheville and surrounding Western North Carolina communities, symptoms may be more noticeable during downhill walking or stair descent.
